Chloroquine is now uncommonly used in favor of its derivative hydroxychloroquine. In the United States, hydroxychloroquine is most often used for its anti-inflammatory effects in rheumatology and dermatology.

What is the generic name for plaquenil How does hydroxychloroquine help lupus

I was just talking to my rheumatologist today. She has prescribed plaquenil. She said very rare side effect is some sort of deposits on the eye. I would contact your dr to be on safe side. My dr said to have eye exam with dilation a few months after starting. Just tell eye dr you take plaquenil and she said they would know exactly what to look for. Many systemic medications may cause retinal toxicity. One such commonly used medication for dermatologic and rheumatologic inflammatory conditions is hydroxychloroquine Plaquenil, a chloroquine derivative. It is used to treat many diseases including malaria, rheumatoid arthritis and systemic lupus erythematosus. Posterior exam showed 1+ nuclear sclerosis of the lenses OU. Vitreous was clear of cell OU. Fundus exam showed OD and OS optic nerves were pink and distinct with a 0.15/0.15 cup-to-disc ratio. The macula in each eye exhibited a flat appearance with absent foveal reflexes. Peripheral retina was unremarkable OU.

While early toxicity may be asymtomatic, patients with more advanced stage of toxicity may complain of color vision changes or paracentral scotomas. Its toxic effects on the retina are seen in the macula.

A recent article published by the American Academy of Ophthalmology AAO presented new recommendations for screening of patients being managed with hydroxychloroquine HCQ. In this article, new recommendations were made that changed the previous monitoring paradigm.

In 2016, the American Academy of Ophthalmology published recommendations on screening for chloroquine and hydroxychloroquine retinopathy. The primary screening tests are automated visual fields plus spectral-domain optical coherence tomography SD-OCT. Anne E. Fung, MD, a retina specialist at Pacific Eye Associates in San Francisco, averages one Plaquenil screening a week, due to her close affiliation with the medical center’s rheumatology and dermatology clinics. Still, in 10 years of practice, she’s seen just one case of toxicity. Dr. Aug 30, 2011 Diagnosis Hydroxychloroquine-induced retinal toxicity Discussion. Chloroquine CQ and hydroxychloroquine Plaquenil HCQ have been used for many years, initially for the treatment of malaria but now more commonly for the treatment of inflammatory diseases such as rheumatoid arthritis and lupus 1. It is now considered for new disease applications in diabetes, heart disease and adjunct cancer therapies 2.

Hydroxychloroquine inhibits autophagy to potentiate. We found that inhibiting autophagy through low-dose oral hydroxychloroquine HCQ administration increases the responsiveness of resistant mammary tumors to antiestrogens. Moreover, we show that the combination of tamoxifen and HCQ is more effective than that of Faslodex and HCQ due to activities within the tumor microenvironment.
