Plaquenil has long been associated with retinal toxicity which can lead to vision loss. In particular it affects the macular region of the retina which is responsible for central vision. Very early on toxicity is usually asymptomatic detected only by a spectral domain optical coherence tomographer and a macular visual field test. The main outcomes were toxicity as determined by characteristic visual field loss or retinal thinning and photoreceptor damage. They also examined risk factors and prevalence. The study findings revealed that when hydroxychloroquine was used at doses 5mg /kg, the risk of retinopathy was 1% in the first 5 yrs. 2% in 10 yrs. 20 % after 20 yrs. Visual fields may be determined by several methods including a tangent screen, Goldmann perimeter, and computerized automated perimeters. Visual field examinations may be indicated in the diagnosis and monitoring visual field loss due to blepharoptosis or to disease involving the cornea, lens, retina, optic nerve, and intracranial visual pathway.

Plaquenil is also an antirheumatic medicine and is used to treat symptoms of rheumatoid arthritis and discoid or systemic lupus erythematosus. This medicine is not effective against all strains of malaria.
