    Metformin with meals

Let me first say, I don’t hate Metformin for women with PCOS. For some women it really does help spur ovulation, control blood sugar and help with some weight management but….it’s not without its share of issues. And it’s definitely not the magic bullet for weight loss – although it’s usually presented that way. Metformin is typically given with meals throughout the day, or more commonly now the extended release version is given once with dinner or at bedtime. While only having to pop a pill one time per day is always appealing, this once a day dosing (especially at bedtime) is where I see the most problems with my patients. It lowers both fasting and post meal glucose levels by decreasing the glucose absorption in your intestines after a meal; as well as decreasing the amount of glucose your liver makes for later use. It also does help improve insulin sensitivity by increasing glucose movement into a cell. Not so fast, here are the most common issues I see in women using Metformin: Metformin is notorious for causing sometimes severe digestive issues including stomach pain or upset, nausea, vomiting, diarrhea and even a sense of body weakness or metallic taste in the mouth in some. This patient is a good example of how real life can affect adherence to therapy. Two main issues for the pharmacist to address in this situation are medication options and dietary habits but he or she could also consider supportive advice in terms of this patient’s employment concerns. Metformin has a large evidence base for reducing both morbidity and mortality and it is, rightfully, first-line therapy in the treatment of type 2 diabetes. This is recommended not only by the National Institute for Health and Care Excellence, but also the American Diabetes Association and the European Association for the Study of Diabetes. so although there is a small risk of hypoglycaemia if taken without food, this is minimal compared with other antidiabetic drugs. Metformin can, however, increase the risk of hypoglycaemia if used in combination with other antidiabetic medicines. The mechanisms above mean that metformin predominantly increases insulin efficacy and so should be taken with meals when endogenous insulin is produced.

A nurse recently told me that metformin doesn't work without food. Although the medication label says “Take medication with food” as well as “Do not drink. Carefully follow the special meal plan your doctor gave you. This is a very important part of controlling your condition, and is necessary if the medicine is to work properly. Also, exercise regularly and test for sugar in your blood or urine as directed. Metformin should be taken with meals to help reduce stomach or bowel side effects that may occur during the first few weeks of treatment. Swallow the tablet or extended-release tablet whole with a full glass of water. While taking the extended-release tablet, part of the tablet may pass into your stool after your body has absorbed the medicine. Measure the oral liquid with a marked measuring spoon, oral syringe, or medicine cup. The average household teaspoon may not hold the right amount of liquid. Use only the brand of this medicine that your doctor prescribed. You may notice improvement in your blood glucose control in 1 to 2 weeks, but the full effect of blood glucose control may take up to 2 to 3 months. Hello all-I was just wondering, should I be taking my Metformin before meals (like 10-20 minutes) or can I take it as I'm eating or just after I've finished? 5 days of fasting and I finally was able to keep something "real" down. I am having execelent results the am is just after eating and the pm is before eating, supper time varies but my other meals and snacks are by the clock. I don't know if it is important to get it "onboard" before I put food in my belly or if I can just take it whenever. In my view, metformin’s primary beneficial effects build up over time. :)Links added by the Tu Diabetes Administration:https://forum.tudiabetes.org/topics/metformin-does-it-matter-whenhttps://forum.tudiabetes.org/topics/how-long-does-metformin-er-last-in-your-system Hi Devon, I take mine, twice a day, at about 12 hour intervals am and pm. You don’t time the medication to obtain the benefits, you time the medication to minimize the side effects. Some people may find that they have lessgastric distress if they take the metformin with food, others minimize distress by taking it without food. I take a maximum dose, 1g in the morning and 1.5g in the evening. Although I have certain gastric issues, metformin does not bother me. One thing that you should be aware of is that one of the effects of metformin is to impede the absorption of carbs and for some people I’ve heard taking metformin with carby meals can amplified some gastric issues. Devon, I’m on Glumetza in lieu of Metformin (TOO MUCH stomach upset for me) and take it twice a day (as of recently in conjunction with insulin)–with breakfast and dinner. I find that if I take it literally at the first bite of food, or midway into the meal, I have less side effects.
