Tegretol-XR is used to treat certain types of seizures (epilepsy). It is also used to relieve pain due to trigeminal neuralgia (tic douloureux) and in the treatment of bipolar disorder (manic-depressive illness). Tegretol-XR works in the brain and nervous system to control seizures, pain, and bipolar disorder. Tegretol-XR is available only with your doctor's prescription. Once a medicine has been approved for marketing for a certain use, experience may show that it is also useful for other medical problems. Although these uses are not included in product labeling, Tegretol-XR is used in certain patients with the following medical conditions: An indication is a term used for the list of condition or symptom or illness for which the medicine is prescribed or used by the patient. For example, acetaminophen or paracetamol is used for fever by the patient, or the doctor prescribes it for a headache or body pains. Now fever, headache and body pains are the indications of paracetamol. Plantain (psyllium seed): inhibited GI absorption of drug Monitor patient closely. • Drug has been linked to aplastic anemia and agranulocytosis. Increase by up to 100 mg weekly until therapeutic levels are reached. Institute seizure precautions if drug must be withdrawn suddenly. • Transient or persistent decreases in platelet or white blood cell (WBC) counts have occurred, but data aren't available to accurately estimate incidence or outcome. • Assess for history of psychosis; drug may activate symptoms. Rarely, leukopenia cases progressed to aplastic anemia or agranulocytosis. • Monitor baseline hematologic, kidney, and liver function test results. • Obtain complete pretreatment hematologic tests as baseline. Increase by up to 200 mg/day q 7 days until therapeutic blood levels are reached. • During dosage adjustments, monitor vital signs and fluid intake and output. If WBC or platelet count drops during therapy, monitor closely. Usual maintenance dosage is 600 to 1,200 mg/day in divided doses q 6 to 8 hours. Stay alert for fluid retention, renal failure, and cardiovascular complications. Consider withdrawing drug if evidence of significant bone marrow depression develops. In children ages 12 to 15, don't exceed 1 g/day. • With high doses, monitor CBC weekly for first 3 months and then monthly to detect bone marrow depression.
Summary Description and Clinical Pharmacology Indications and Dosage Warnings and Precautions Side Effects and Adverse Reactions Drug Interactions, Overdosage, Contraindications, Other Rx Info Active Ingredients Side Effect Reports APLASTIC ANEMIA AND AGRANULOCYTOSIS HAVE BEEN REPORTED IN ASSOCIATION WITH THE USE OF TEGRETOL. DATA FROM A POPULATION-BASED CASE CONTROL STUDY DEMONSTRATE THAT THE RISK OF DEVELOPING THESE REACTIONS IS 5-8 TIMES GREATER THAN IN THE GENERAL POPULATION. HOWEVER, THE OVERALL RISK OF THESE REACTIONS IN THE UNTREATED GENERAL POPULATION IS LOW, APPROXIMATELY SIX PATIENTS PER ONE MILLION POPULATION PER YEAR FOR AGRANULOCYTOSIS AND TWO PATIENTS PER ONE MILLION POPULATION PER YEAR FOR APLASTIC ANEMIA. ALTHOUGH REPORTS OF TRANSIENT OR PERSISTENT DECREASED PLATELET OR WHITE BLOOD CELL COUNTS ARE NOT UNCOMMON IN ASSOCIATION WITH THE USE OF TEGRETOL, DATA ARE NOT AVAILABLE TO ESTIMATE ACCURATELY THEIR INCIDENCE OR OUTCOME. HOWEVER, THE VAST MAJORITY OF THE CASES OF LEUKOPENIA HAVE NOT PROGRESSED TO THE MORE SERIOUS CONDITIONS OF APLASTIC ANEMIA OR AGRANULOCYTOSIS. BECAUSE OF THE VERY LOW INCIDENCE OF AGRANULOCYTOSIS AND APLASTIC ANEMIA, THE VAST MAJORITY OF MINOR HEMATOLOGIC CHANGES OBSERVED IN MONITORING OF PATIENTS ON TEGRETOL ARE UNLIKELY TO SIGNAL THE OCCURRENCE OF EITHER ABNORMALITY. NONETHELESS, COMPLETE PRETREATMENT HEMATOLOGICAL TESTING SHOULD BE OBTAINED AS A BASELINE. Carbamazepine is also sometimes used to treat mental illnesses, depression, posttraumatic stress disorder, drug and alcohol withdrawal, restless legs syndrome, diabetes insipidus, certain pain syndromes, and a disease in children called chorea.
